Hypertensive crisis or not?

by Adam

 Question:

10. Which of the following would be considered a priority patient?
Your Answer: A 34 year old man who was stung by several bees
Incorrect

Correct Answer is: A 55 year old female with a blood pressure of 178/90

A blood pressure of 178/90 would be a hypertensive crisis and would be considered a priority patient.

This blood pressure of that, unless rapidly increased, is not high enough to constitute a hypertensive crisis. A diastolic of over 100 is the accepted lower limit.

Answer:

When a systolic blood pressure reaches anywhere above 120 it is considered hypertension. However we know that most people live with pressures much higher than this. With a systolic pressure of 178 this is pretty significant and you better be getting ready to transport. This pt. could be getting ready to stroke out. Also you're correct the diastolic does look good, but the systolic is more worrysome.

With your answer and the bee stings, think about the guy who is not allergic to bees. Now if the 34 y/o male was symptomatic that is a different story. Remember that the NREMT tests want you to choose the BEST answer so that is why our questions are the way they are.
